2) Tuberculosis-TB-Screening-Questionnaire

3) Insurance waiver   – All students are required to enroll or waive the health insurance annually. The waiver site opens June 3, 2021 and closes September 9, 2021. Any student that does not waive will be automatically enrolled and charged the annual fee of $3,222.00 for the 21-22 academic year.

4) Medical supplies to bring to campus– Essential items every student should bring to campus.
