Note: For some student teaching placements, the cooperating teacher serves as the teacher candidate’s University Supervisor. The role title for these individuals is Cooperating Teacher-Supervisor.

Spring 2022 Student Teaching Dates –  January 25, 2022 through May 6, 2022.

Complete this form by using the link above.

Mid-Term Assessment: Teacher Disposition Assessment Form 

Submit the assessment to the university using the link above after reaching consensus at three way conference with teacher candidate and cooperating teacher.

Full Semester

First Student Teaching Classroom Observation Form submitted by February 18, 2022

Second Student Teaching Classroom Observation Form submitted by March 11, 2022

Third Student Teaching Classroom Observation Form submitted by April  8, 2022

Fourth Student Teaching Classroom Observation Form submitted by May 6, 2022

1 at Mid-Semester by March 11, 2022

1 at End of Semester by May 6, 2022

Consensus assessment at mid-semester after three way conference with student teacher, cooperating teacher and University Supervisor.

Dual Placements

 

First 2 observations during the first half of the semester and final 2 observations during the second half of the semester following the schedule above.

For placements in first seven weeks: one close to Mid-Semester by March 11, 2022

For placements in second seven weeks: one close to End of Semester by  May 6, 2022

Consensus assessment at mid-semester after three way conference with student teacher, cooperating teacher and mentor only for first 7 week placement by March 11, 2022

4 weeks Modified Student Teaching for Special Education

  None 1 – at midpoint and 1 – at the end of placement.  None

Note: Additional observations may be required for adequate supervision
